Engaging Patients of Color to Advance Health Equity Learning Collaborative

ATW Health Solutions partnered with the Center for Health Care Strategies (CHCS), with support from the New York Health Foundation, to carry out a 12-month-long learning collaborative with New York-based healthcare organizations. This work included:

• Health equity and transformation

• Technical support

• Communications

• Report writing and synthesis

• Learning networks/education dissemination

• Stakeholder engagement/patient engagement

• Program design/program management

ATW team members served as national subject matter experts on engagement and equity, providing thought leadership and coaching to 7 healthcare and community-based organizations selected to participate in a learning collaborative to promote more meaningful partnerships with patients and communities of color through existing or new mechanisms, including patient ambassador programs, advisory councils, and digital engagement tools; develop an action plan to use patient engagement strategies more effectively within internal health equity improvement efforts, such as quality improvement and patient-centered measurement; and Sustain and expand these efforts by integrating patient engagement strategies into existing organizational structures and mechanisms.
